Karlka Recruiting Group has partnered with a Federal Government Agency to engage an APS6 Clinical Care Coordinator. This position is located in their busy Surry Hills, Sydney NSW office for an initial 24-month contract.
  
About the Position
The Clinical Care Coordinator will work within a multidisciplinary team to provide mental health and wellbeing support to clients and their families.

The role will provide short-term, goal-oriented clinical case management to clients with complex health and psychosocial needs, supporting them to access and meaningfully engage with appropriate mental health care. The position involves care coordination, brief intervention, risk management, referral support and stakeholder engagement across internal and external service networks.

Duties

  • Provide evidence-informed clinical services, including risk management, brief intervention, care coordination and referral support
  • Manage a caseload of clients with complex and/or multiple needs
  • Maintain confidential client records, including progress notes, care plans, risk assessments and client interactions
  • Liaise with internal teams, government agencies, community services, mental health providers and other key stakeholders
  • Support clients to identify appropriate services, reduce barriers to care and improve engagement with mental health supports
  • Participate in case reviews, clinical supervision and Continuing Professional Development activities
  • Support group programs, psychoeducation, community development, service promotion and program improvement activities as required
  • Conduct intake assessments and assist with allocation to appropriate services where required

Requirements/Skills

  • Experience providing clinical care coordination, case management, counselling, mental health support or psychosocial support services
  • Strong understanding of risk assessment, brief intervention, referral pathways and client-centred practice
  • Ability to work with clients experiencing complex health, mental health and psychosocial needs
  • Strong stakeholder engagement skills, including the ability to work with community services, government agencies and clinical providers
  • Ability to maintain accurate and confidential client records
  • Australian citizenship required
  • Ability to obtain and/or maintain a Baseline security clearance
  • Ability to obtain a Working with Vulnerable People registration or Working with Children Check
  • One of the following is required:
    • Registered Psychologist, Occupational Therapist or Registered Nurse with full AHPRA registration
    • Social Worker with AASW membership
    • Masters-level counselling qualification with PACFA registration as a Registered Clinical Counsellor, or ACA Level 3 or Level 4 registration
